Team planning day

Our Coordination of Support Team met yesterday for their annual Team Planning Day in Palmerston. Team members from Alice Springs also travelled up for the event.

Somerville’s Coordination of Support Team provides services to people with disabilities and their families across the Northern Territory. The day focused on team discussions, quality improvement, and service development.

It was also an opportunity to celebrate the many achievements the team has made.

Our Coordination of Support Team continues to support participants to successfully:

– Access much-needed allied health services and support.

– Return to Country and travel to support their goals and maintain a connection with family and friends.

– Gain access to SIL services and live a more independent life.

– Lead an independent life and have choice and control.

– Have their homes modified to suit their needs and increase their participation and accessibility within their own home.

– Transition from hospital into the community with the support they need.

– Gather and prepare evidence for NDIS Plan reviews.
